Please do not send medical details through the contact form. We will talk with you about your health by phone or in person at your visit.

What this website collects

When you send a message, we receive what you enter in the form: your name, email address, phone number, and your message. New patient intake forms open on a separate, secure patient system, which is covered by its own privacy policy and our Notice of Privacy Practices.

Our form uses a security check from Google (reCAPTCHA) to block spam. It looks at basic technical details such as your IP address and how you use the page, under Google’s own privacy policy.

Like nearly every website, our website platform automatically processes basic technical information, such as your IP address and browser type, to deliver pages, load fonts, and keep the site secure. It also sets a small number of cookies needed for security and to count visits in general terms.

Analytics

To understand how people find and use this website, we use Google Analytics. It sets cookies in your browser and collects information such as the pages you visit, how long you stay, the site that sent you here, your general location, and your device and browser. It never receives what you type into the contact form. Google processes this information for us under Google’s privacy policy. We see it only as totals and trends, and we do not use it to identify you.

How we use it

We use what you send only to reply to you and to schedule your visit. We do not sell, rent, or trade your information to anyone, and we do not use advertising trackers. We may share information if the law requires it.

Children

This website is meant for adults. We do not knowingly collect information online from children under 13.
